HUBUNGAN FILTRASI GLOMERULUS DENGAN KADAR BIKARBONAT PLASMA PADA PENYAKIT GINJAL KRONIK STADIUM 5 DENGAN ASIDOSIS METABOLIK

Abstract


ABSTRAK

Kadar bikarbonat plasma yang rendah berhubungan dengan tingginya mortalitas pada pasien PGK stadium lanjut, Beratnya derajat hipobikarbonatemia berkaitan dengan derajat laju filtrasi glomerulus (LFG) dimana semakin rendah LFG maka semakin rendah pula kadar bikarbonat plasma. maka berdasarkan adanya hubungan bahwa derajat hipobikarbonatemia berkaitan dengan derajat LFG, penelitian ini bertujuan ingin mengetahui lebih lanjut peran diagnostik nilai LFG dalam memperkirakan kadar bikarbonat plasma pada PGK stadium 5 dengan asidosis metabolik.Metode, penelitian ; Populasi penelitian adalah semua pasien yang menderita PGK stadium 5 dengan asidosis metabolik yang dirawat di Bagian Ilmu Penyakit Dalam, RS Dr. M. Djamil. Dari hasil penelitian diperoleh pria lebih banyak menderita PGK stadium 5 dengan asidosis metabolic,Kesimpulah; Laju Filtrasi Glomerulus (LFG) tidak dapat dijadikan alat diagnostik untuk memperkirakan kadar bikarbonat plasma kurang dari 12 mmol/l pada PGK stadium 5 dengan asidosis metabolik.Kadar bikarbonat plasma pasien PGK stadium 5 dengan asidosis metabolik memiliki kadar rerata 9,1 (SD 3,4) mmol/l dengan estimasi selang 8,2 - 10,0 mmol/l.Tidak terdapat hubungan bermakna antara nilai LFG dan kadar bikarbonat plasma.Terdapat korelasi positif antara kadar bikarbonat plasma dan nilai base excess pada PGK stadium 5 dengan asidosis metabolik.

ABSTRACT

Low plasma bicarbonate levels are associated with high mortality in patients with advanced CKD. The severity of the degree of hypobycarbonatemia is related to the degree of glomerular filtration rate (GFR) where the lower the GFR, the lower the plasma bicarbonate level. Therefore, based on the relationship that the degree of hypobycarbonatemia is related to the degree of GFR, this study aims to find out more about the diagnostic role of GFR in estimating plasma bicarbonate levels in stage 5 CKD with metabolic acidosis. Method, study; The study population was all patients suffering from CKD stage 5 with metabolic acidosis who were treated at the Internal Medicine Department, Dr. M. Djamil. The results showed that men suffered more from CKD stage 5 with metabolic acidosis. Kesimpulah; Glomerular Filtration Rate (GFR) cannot be used as a diagnostic tool to estimate plasma bicarbonate levels of less than 12 mmol / l in stage 5 CKD with metabolic acidosis. Plasma bicarbonate levels in CKD stage 5 patients with metabolic acidosis had a mean level of 9.1 (SD 3, 4) mmol / l with an estimated range of 8.2 - 10.0 mmol /l. There was no significant relationship between LFG values and plasma bicarbonate levels. There was a positive correlation between plasma bicarbonate levels and base excess values in stage 5 CKD with metabolic acidosis.
